Do I need a referral form to see Dr. Pelavin?

Some HMO insurances require patients to have a referral from their primary care physician in order to be seen. Obtaining the referral is the patient's responsibility. NOTE: Each appointment requires a new referral. Our office participates with HAP, Blue Care Network, Aetna HMO, in addition to several others.

What is your cancellation policy?

We require at least 24-hour notice when canceling or rescheduling an appointment. Your courtesy allows us to be of service to other patients.

What if I miss my appointment?

If you forget to cancel your appointment, there will be a charge incurred. If this is a regular occurrence our office may not be able to make further appointments for you.
